Sacramento Fire Department personnel look forward to assisting you with your request. Please follow these steps to request fire and/or medical incident records:
TO REQUEST A FIRE INCIDENT REPORT
As of April 1, 2014 all request for Fire Incident Reports will be directed to the City of Sacramento’s City Clerk’s Office. While a written request is not required, the City prefers that all public record requests be in writing so that City staff can more accurately identify the records sought and process the request more efficiently. Please be as specific as possible when making your request; include dates and times if applicable, and any details regarding the information you are requesting. For your convenience, these requests may be made by any of the following methods:

TO REQUEST MEDICAL INCIDENT RECORDS (will include related fire incident report)
Click here to download and print a request form and complete the request form to the best of your ability. Be sure the request form is dated and signed by the patient. If the patient’s signature is not available, legal documentation such as a subpoena, power of attorney, or letter of representation is required and must accompany the request.
The cost for a medical incident report is $5.00. Acceptable forms of payment are checks or money orders made payable to Sacramento Fire Department. Please do not send cash. Return or mail the request, with payment, to:
